In practice

Step one: ask which fields people always care about when they fill one in. Step two: ask which fields they never use. Step three: ask what important information goes into the notes because there is nowhere structured for it. Step four: ask whether each required field is truly required, or sometimes left blank. Step five: capture additions, removals and renames, and clarify meanings, such as harm level versus severity. Spend twenty to twenty-five minutes here, and watch especially for compliance items, such as a field that must be tracked for regulatory reporting.
