Building a Pack: Header to Properties

Captions available

The idea

The book builds the Incident Report Pack in ten steps, and the first three come mostly from phases one and two.

In practice

Step one, header and definition: from the inventory, the Incident Report is a data record whose primary zone is intake. Its definition is a record of a safety event, written so that the event can be investigated, tracked and reported. Record synonyms and scope, for example near misses included and equipment-only events excluded, if that is true in your setting. Step two, purpose: write in simple language why the thing exists and which outcomes it supports. Step three, key properties: consolidate the field lists from the cards into groups, such as reporter, event, patient and metadata. Give each field its meaning, whether it is required, and its rules.

Where people slip

List only the properties that matter for decisions, reports, audits and policy. The full schema belongs in a data dictionary, not the Pack.
