Building a Pack: Obligations, Questions, Examples

The idea

The last three steps of building a Pack tie it to policy, make its uncertainty visible, and ground it in a real instance.

In practice

Step one, constraints and obligations: say which policies require the thing to exist, which fields compliance needs, how long it must be kept, and who may see or change it. Step two, open questions and risks: write down what is undecided. Is a near miss its own thing, or an incident report with a flag? May reporters edit after submitting? How are incidents across several locations handled? Step three, examples: fill in at least one realistic instance, such as a staff nurse reporting a patient fall in a named unit, harm moderate, status submitted.

Where people slip

The open questions do not need answers yet, but they need a home. Examples help testers, newcomers and AI tools generating test data.
