The Case: The Pack
The idea
The case's phase three assembles the Incident Report's Pack with AI help, then lets the people who do the work correct it.
In practice
Step one: the analyst and the data architect assemble a first draft, with AI merging field lists from the cards and suggesting a lifecycle of new, submitted, under review, closed and archived. Step two: the charge nurse and quality manager add the returned-for-more-information state, and a new card. Step three: the team chooses Gold, because the Incident Report is central to patient safety, sensitive for regulators and likely to be touched by AI. Step four: they fill every Gold section: transition rules, constraints and retention, and usage across screens and reports. Less critical things, such as shift schedules, stay Bronze or Silver.
Where people slip
The Gold decision came from the three risk questions, not from the thing's importance in daily work.
