The Case: Validation

The idea

The case's phase four is one review session on the Incident Report alone, with nurses from three units, quality, safety, data and testing.

In practice

Step one, definition: a nurse notes that minor reports are often filed only for trends, so the definition is widened beyond investigation. Step two, fields: harm level may start as unknown, with a new card for updating it during review, and a regulatory-reportable flag is added. Step three, lifecycle: closed is reworded to mean the safety review is complete, and follow-up tasks move to the Action Plan. Step four, cards: the safety officer catches the over-general reporting rule, and the team rebuilds it from incident type, harm level and the policy version.

Where people slip

Every change came from someone who does the work. Rewording closed kept the Incident Report from becoming a dumping ground for every workflow detail.
