Self-Retaining Retractors
The idea
Self-retaining retractors hold a wound open on their own, using a ratchet or a frame with adjustable blades, so no one has to hold them.
In practice
Learn the main types in order of size. First, small self-retaining retractors, such as the Weitlaner, hold open small or shallow wounds with toothed arms locked by a ratchet. Second, large abdominal self-retainers, such as the Balfour, give retraction in several directions for big abdominal incisions. Third, table-mounted systems, such as the Bookwalter, attach to the operating table and carry multiple adjustable blades. Every blade, screw and part of a multi-part retractor is counted as a separate item.
Where people slip
Multi-part retractors are a common source of count problems, because blades and screws can separate during the case. Know how many parts went into the wound, and confirm they all came out.
