When Counts Happen

Captions available

The idea

Counts are done at set points in every case that requires them, at least at the beginning and the end.

In practice

Learn the usual points, as the witnesses describe them. First, before the procedure begins, which establishes the baseline. Second, before closing a cavity within a cavity, such as the uterus within the abdomen. Third, before wound closure begins, at the first layer of closure. Fourth, at skin closure. Instruments are usually counted at the beginning and before wound closure. Fifth, whenever the scrub person or circulating nurse is permanently relieved during the case. Items added during the case are counted as they arrive and added to the record.

Where people slip

The exact count points are set by facility policy. When a closed incision is reopened after the final count, the closing count is repeated.
