The idea
Counts are recorded by the circulating nurse as correct or incorrect, with the names of the people who counted.
In practice
The record also notes any items intentionally left in the patient, any discrepancy and the actions taken, and the reason if a required count was not done.
Where people slip
Many teams also track counts on a whiteboard during the case, as facility policy allows.
