The idea
When the scrub person or circulating nurse is replaced during a case, responsibility for the count is handed over deliberately.
In practice
Ideally the same two people do all counts; when someone is permanently relieved, a count and a transfer of information follow facility policy.
Where people slip
The person arriving should know where every counted item is before the person leaving goes.
