Reporting an Incident
The idea
An incident report records an event that harmed or could have harmed a patient, visitor or staff member, so that the facility can learn from it.
In practice
Follow the purpose of a report step by step. First, the event is reported promptly, through the facility's reporting system. Second, the report describes what happened, factually, without blame or speculation. Third, the facility analyzes reports to find system causes, such as confusing equipment or unclear policies, rather than to punish individuals. Fourth, changes are made and shared, so the same event is less likely to recur. A culture of safety depends on this cycle: open communication, error reporting, and continuous improvement.
Where people slip
Reporting is not an accusation. A report about your own error, made honestly, is part of professional practice, and a system that punishes honest reports only stops learning.
