The idea
Suture needles are sharps, counted by the number marked on each package and kept contained throughout the case.
In practice
Needles are verified when packages are opened, kept in a needle counter, on a needle holder or in their package, and never left loose.
Where people slip
Many needles passed back and forth make needle counts error-prone, so every used needle goes straight into the counter.
