Choosing Closure for the Tissue

Captions available

The idea

The surgeon chooses suture, needle and closure method to fit the tissue and the purpose of the closure.

In practice

Follow the factors in the order they are usually weighed. First, the purpose: holding tissue together until it heals, or permanently fixing a structure or device. Second, the tissue: tough tissue such as fascia needs strong suture and often a cutting or heavy needle; delicate tissue such as bowel or vessels needs fine suture and a taper needle. Third, the material's properties: strength, absorption, tissue reaction and handling. Fourth, the size, matched to the tissue's strength. Fifth, for skin, the choice between suture, staples and glue, which often reflects cosmetic outcome and the surgeon's preference.

Where people slip

The scrub person does not choose the suture, but understanding why a surgeon chooses it lets the scrub person anticipate the next request, and notice when a package opened does not match the layer being closed.
