Procedures and Specimens

The idea

Procedures and specimen collections follow the order, the scope rules and the labeling rules every time.

In practice

Step one: confirm the order and that the task is within your role and training. Step two: gather supplies and use the right technique, whether for a dressing, a swab, a urine sample or a blood draw in the correct order. Step three: label each specimen at the point of collection, matching the requisition. Step four: package and send specimens as the laboratory requires. Step five: document what was done, and report any complication.

Where people slip

Every specimen depends on an accurate label. A perfectly collected sample with the wrong name on it is worse than no sample, because nobody downstream has any reason to question it. Label at the point of collection, every time, and recheck against the requisition before anything leaves the room.
