Whole Visit: A Child With a Sore Throat

Captions available

The idea

This visit is constructed for this course. It follows a seven-year-old with a sore throat and fever from the first phone call to checkout.

In practice

A parent calls about a child with a sore throat and fever. The medical assistant triages the call using the practice's guidelines and books a same-day slot from the buffer time. At check-in, she offers the child a mask, because he is coughing, and seats the family away from others. In the exam room, she confirms the child's identity with his parent, asks about allergies and medications, and measures his vital signs. She takes his temperature by a route suited to his age, records the route, and counts his breathing for a full minute. His temperature is above the normal range, and she reports it to the provider. The provider examines him and orders a rapid strep test. She labels the container, has the parent help hold the child still, and swabs the tonsils and the back of the throat without touching the tongue or teeth. She runs the test following the manufacturer's instructions and records the result exactly as read, with the time, then reports it to the provider. The provider explains the result and the treatment plan to the parent. At checkout, she reinforces the provider's instructions and asks the parent to explain them back. Then she schedules any follow-up the provider requested.

Where people slip

Nothing in this visit was unusual, which makes it a good model. Triage, infection control, age-appropriate vital signs, careful collection, exact recording and teach-back each did their part, and the provider made every clinical decision. The parent left knowing what to do at home because the instructions were checked, not assumed.
