Identify the Patient First
Captions available
The idea
Before any care, confirm the patient's identity with at least two identifiers, following the practice's policy.
In practice
Common identifiers are the patient's full name and date of birth, checked against the record or the order in front of you.
Where people slip
Check the identifiers against the record every time, even for a patient you know well.
