The idea
Information gathered about a patient is either subjective or objective.
In practice
Subjective data is what the patient reports, such as "I have a headache"; objective data is what you observe or measure, such as a temperature or a visible rash.
Where people slip
Record subjective data in the patient's own words, and objective data as measured.
