The Case: Mapping

The idea

The case's phase one is a ninety-minute workshop run by the lead analyst, following the book's workshop steps.

In practice

Step one, the noun storm: the request is for things, not steps or buttons, and the notes fill with Incident Report, Near-Miss Event, Location, Action Plan, Task, Regulatory Report, Root Cause Analysis, Audit Log and more, duplicates included. Step two, five zones: intake, investigation, improvement and follow-up, reporting and analytics, and compliance and policy. Step three, a rough map: an Incident Report may create an Investigation Case, which produces a Root Cause Analysis, then an Action Plan with many Tasks. Step four, five core things. Step five, AI writes a one-page narrative of the map, and the analyst deletes an invented risk-score field.

Where people slip

The safety officer's verdict: the map is already better than the last forty-page specification he saw.
