A Worked Card: Rules and Example

The idea

The second half of the book's worked card holds the rules, the location, the traceability and an example.

In practice

Step one, validation: the incident time cannot be in the future, the location must be active, the type comes from the configured list, and the description has a minimum length. Step two, conditional rules: if a patient was involved, the patient reference and harm level become required. Step three, permissions and timing: only signed-in staff may create reports, and harm incidents entered more than a day late are flagged for review. Step four, location and traceability: the internal web form; the safety reporting policy; the Incident Report Pack; related cards. Step five, example: a nurse reports a fall with injury in a named unit, and the form will not accept the report until she selects the patient and describes the event.

Where people slip

One card is not a specification, but it carries enough structure to guide screen design, drive validation, generate tests, and keep an AI tool inside its fields.
