The book's worked card, REQ, INC, zero one, Create Incident Report, shows the required sections in use. Its policy and personal details are illustrative.
In practice
Step one, business statement: frontline staff must be able to record safety events, so that quality, risk and regulatory staff can investigate, find trends and meet reporting duties. Step two, material focus: the primary thing is the Incident Report, with the Patient Reference, Location and Incident Type as related things. Step three, change type: create. Step four, properties, grouped. Reporter: reporter identity and role, both required. Event: date and time, location, type and description, all required, plus a patient-involved flag. Patient: a patient reference, required only if a patient was involved. Metadata: the created date and user, filled automatically.
Where people slip
Grouping the properties is what makes a long field list readable, and it tells designers where each field belongs.
