Laparoscopy Basics

The idea

Laparoscopy is minimally invasive surgery in the abdomen, using a camera and long instruments passed through small ports.

In practice

Follow how a laparoscopic case is set up and run. First, working space is created by filling the abdomen with carbon dioxide gas, called insufflation. Second, ports, called trocars, are placed through small incisions in the abdominal wall. Third, the laparoscope, with its camera and light, is inserted to show the inside of the abdomen on a monitor. Fourth, long instruments such as graspers, scissors, dissectors and needle holders work through the other ports. Fifth, at the end, the instruments and ports are removed, the gas is released, and the small incisions are closed. Common laparoscopic procedures include gallbladder removal, hernia repair and appendectomy.

Where people slip

Many laparoscopic instruments have joints, lumens and insulation that need careful handling and inspection, and an open set is kept ready in case the case must convert.
