Incisions and Exposure

The idea
In open surgery, the incision is placed and sized to give the surgeon the exposure the operation needs.
In practice
A laparotomy, for example, opens the abdominal wall through its layers to reach the abdominal cavity, and retractors hold it open.
Where people slip
Every layer opened must be closed again, which is why the scrub person follows the layers on the way in.
