Prep and Drape for a Laparotomy
The idea
A worked example, constructed for this course, follows the steps from positioning to incision for an open abdominal operation.
In practice
The patient is supine on the table, and the circulating nurse places the dispersive pad on the thigh, over a large muscle away from bone and scars. The surgeon decides no hair removal is needed. The circulating nurse prepares the abdomen with the facility's alcohol-based chlorhexidine solution, beginning at the planned incision along the midline and working outward, without returning to the center with a used applicator. The circulator watches for pooling at the patient's sides and removes a towel that has soaked up solution. The label calls for a set drying time; the team waits for it fully, and the scrub person, Reese, uses the time to recheck the Mayo stand. When the skin is dry, Reese and the surgeon apply the sterile drapes as the facility's draping procedure directs, exposing only the planned incision area. Reese secures the suction tubing and electrosurgical cord with a non-penetrating holder and passes their ends off the field to the circulating nurse, who connects them. The pencil goes into its holster. The smoke evacuator tubing is new. Then the circulating nurse calls for the time out. The site mark is confirmed and visible within the exposed area; the procedure is read from the consent; everyone confirms. The surgeon asks for the knife.
Where people slip
Most of this sequence is waiting and checking: waiting for the prep to dry, checking for pooling, checking the mark. That is where its safety comes from. The scrub person's part, a ready field, secured lines and a holstered pencil, means the case can start the moment the checks are done.
