What a Medication Order Contains

The idea
A complete medication order names the patient, the date and time, the drug, how much, how often, the route, and the prescriber.
In practice
When it matters, an order also gives the patient's age and weight, the exact strength, the quantity or duration, and specific instructions.
Where people slip
An order missing any needed element is not ready to carry out; ask the provider to complete it.
