The Incident Report's Four Causes
The idea
The book works a hospital's Incident Report through all four causes, to show where AI helps and where it misleads.
In practice
Step one, final cause: the report exists so that safety events are recorded, investigated, analyzed for trends and reported to regulators. Step two, efficient cause: frontline staff create it on the reporting screens, and some details arrive from other clinical systems. Step three, formal cause: it links to a patient reference, a location, an action plan and a regulatory report, and moves from new to submitted, under review and closed. Step four, material cause: list its fields, including incident date and time, location, type, description, severity, harm level, a patient-involved flag, status and an audit log. The book labels the field names illustrative.
Where people slip
Without step four written down, a tool may omit harm level or the patient-involved flag. With it, the same tool can draft cards and test cases inside those fields.
