Why Breaks Matter: From Contamination to Infection

Captions available

The idea

A break matters because it can supply the missing link in the chain of infection, carrying microbes into an open wound.

In practice

Trace the path in order. First, a sterile item touches something unsterile and picks up microbes. Second, the item reaches the wound, which is a portal of entry the operation created. Third, the patient, weakened by illness and surgery, is a susceptible host. Fourth, if the microbes multiply, a surgical site infection may develop days later, often after discharge. Fifth, nobody can usually trace the infection back to the moment of contamination. Correcting a break at once removes the link before it connects.

Where people slip

The people who cause a surgical site infection rarely see its result. That is why a break is judged at the moment it happens, by the rule, and not later, by whether the patient happened to get an infection.
