Needle Holders

Captions available

The idea

A needle holder, also called a needle driver, is a ratcheted instrument designed to grip a suture needle firmly while it passes through tissue.

In practice

Learn the common types and how to choose. First, the Mayo-Hegar needle holder is the general-purpose type, used for medium to heavy needles, such as for closing fascia. Second, smaller needle holders, such as the Webster, hold fine needles for delicate tissue. Third, very fine spring-handled needle holders, such as the Castroviejo, are used for eye surgery and microsurgery. Fourth, length is chosen by the depth of the wound: a deep wound needs a longer needle holder. Fifth, the needle is gripped in the jaws at a right angle, about a third of the way back from where the suture attaches.

Where people slip

A needle holder that is too large for its needle can bend or damage the needle. Match the holder to the needle the surgeon will use, and load it before it is asked for.
