Tech in Surgery Prep / Unit 7 TSC 167 worked example, 60 to 90 seconds
Checking the Internal Indicator at Opening
The idea
When a set is opened, the internal chemical indicator is checked before any instrument is used.
In practice
Follow the moment of opening. First, the set is opened with sterile technique. Second, before anything is lifted out, the scrub person finds the internal indicator. Third, the scrub person compares its color with the pass reference for that indicator. Fourth, if it has passed, the instruments may be used. Fifth, if it has not changed fully, or is missing, the whole set is treated as not sterile: it is not used, it is set aside away from the sterile field, and the circulating nurse obtains a replacement and reports the finding to the processing department.
Where people slip
An internal indicator that failed is a sign the sterilant may not have reached the instruments. The cost of opening another set is time; the cost of using a set that was not sterile could be an infection nobody can trace.
