The idea
A sterilizer can sterilize only the surfaces its steam actually reaches.
In practice
Hinged instruments are processed open, items with removable parts are taken apart, and concave items are placed so water drains away.
Where people slip
A closed hinge or a stacked basin can leave a surface the steam never touched.
