Tech in Surgery Prep / Unit 26 TSC 628 worked example, 60 to 90 seconds
Closing in Layers
The idea
A wound is closed in layers, in the reverse order of the layers opened, so that each tissue is brought back together.
In practice
Follow an abdominal closure from deep to surface. First, the deepest layer, the peritoneum, may be closed. Second, the muscle layer and its connective tissue sheath, including the midline, are closed. Third, the subcutaneous fatty layer may be closed or left, as the surgeon judges. Fourth, the skin is closed with suture, staples or surgical glue. Fifth, the dressing is applied. Each layer has its own suture, chosen by the surgeon for that tissue.
Where people slip
Knowing which layer comes next lets the scrub person pass the right suture before it is asked for. A suture meant for skin handed up for fascia, or the reverse, slows the closure and can weaken it.
