Tech in Surgery Prep / Unit 25 TSC 609 worked example, 60 to 90 seconds
Converting to Open
The idea
A minimally invasive operation may be converted to an open operation when the surgeon decides it cannot be completed safely through the ports.
In practice
Follow the team's response in order. First, the surgeon announces the conversion. Second, the open set, kept ready but unopened, is opened by the circulating nurse. Third, the scrub person sets the open instruments, laparotomy pads and suction, adding new items to the count as they are opened. Fourth, ports and laparoscopic instruments are removed and accounted for. Fifth, the procedure continues open, and the final count includes everything used in both parts of the case.
Where people slip
A conversion is often urgent, and urgency is when counts go wrong. Every item opened for the open part is counted in, and every laparoscopic item is counted out.
