A Conversion Mid-Case

The idea

A worked example, constructed for this course, follows a laparoscopic case that converts to open surgery.

In practice

A laparoscopic gallbladder removal becomes difficult: the anatomy is unclear and there is steady bleeding that is hard to see past. The surgeon decides to convert to open and says so. Casey, the scrub person, asks the circulating nurse to open the open set and laparotomy pads, which were in the room. The circulator opens them, and Casey and the circulator count the new pads and the new instruments in before they are used. The surgeon removes the laparoscopic instruments; Casey receives each one and accounts for it, and the ports are removed and set aside. The surgeon makes an open incision below the ribs on the right, and Casey passes retractors, suction and pads. The bleeding is controlled with clamps and ties, and the gallbladder is removed through the open incision. At closing, Casey and the circulating nurse count everything from both parts of the case: the laparoscopic instruments and ports, and the open set, pads and sharps. The count is correct. The circulating nurse records the conversion, so the procedure actually performed is confirmed at the end of the case.

Where people slip

The conversion went smoothly because the open set was ready and the count was extended, not restarted. When a case changes shape, the record must follow it, which is why the final checklist confirms the procedure that was actually done. Counting the laparoscopic items out and the open items in, rather than starting a fresh count, kept a single unbroken record of everything that entered the patient. That continuity is what made the final count meaningful.
