Responding to a Break

Captions available

The idea

When a break happens, the team responds at once and in a consistent way, whoever caused it.

In practice

Follow the response in order. First, stop the action that caused the break. Second, say it aloud so the team knows. Third, identify everything that was contaminated, including anything the contaminated item has touched since. Fourth, remove or cover what was contaminated and replace it with sterile items, according to facility procedure. Fifth, re-establish the sterile field and confirm it is intact. Sixth, report the event if the facility requires it.

Where people slip

The response does not depend on how likely contamination seems. A brush that might not have mattered is handled the same way as one that plainly did, because the difference cannot be seen.
