Instrument Sets and Trays

The idea

Instruments are grouped into standardized sets, or trays, so the same instruments arrive for the same kind of procedure.

In practice

Learn how sets are organized. First, a general or basic set holds the common instruments used in many open procedures. Second, specialty sets hold instruments for particular procedures, and implant systems may arrive on several trays from a vendor. Third, each set should be standardized, with the same type and number of instruments, and come with a tray list used for counting. Fourth, instruments a surgeon uses only occasionally may be packaged individually in peel packs or kept on backup trays rather than in every set. Fifth, teams periodically review trays and remove instruments that are rarely used.

Where people slip

The tray list is the starting point for the instrument count. If a set does not match its list when opened, the circulating nurse is told before the case begins, so the count is correct from the start.
