The idea
Insert the needle with the bevel facing up, at a shallow angle in line with the vein.
In practice
One source gives about thirty degrees to the arm; another says thirty degrees or less; enter swiftly, then follow the vein at the easiest angle.

A steep angle can pass through the vein entirely.
