Watching the Field

Captions available

The idea

Anticipation depends on watching the surgical field continuously and understanding what the surgeon is doing.

In practice

Learn what to watch for, in order. First, the stage of the procedure: opening, exposure, the main work, or closing. Second, the tissue being handled, which suggests the instrument family needed next. Third, bleeding, which calls for sponges, suction, clamps or energy. Fourth, the surgeon's hands: an empty hand turning toward you usually means an instrument is wanted. Fifth, the field itself: smoke, fluid or clutter that needs suction or tidying. Sixth, the count: which counted items are in the wound right now.

Where people slip

Watching the field also means watching for breaks in sterile technique. The scrub person is often the only person looking at the whole field at every moment.
