Tracing One Infection Backwards

The idea

A worked example, constructed for this course, traces a surgical site infection back through the chain of infection to show where each link could have been broken.

In practice

Ten days after a hernia repair, a patient returns with a red, swollen, draining incision, and a culture grows a staphylococcus. The infection-prevention team works backward through the chain. The causative agent is a staphylococcus, the kind commonly found on skin. The portal of entry was the incision. The susceptible host had diabetes, which weakens defenses. The open question is the mode of transmission and the reservoir. The team checks the record. The skin was prepared with an antiseptic and allowed to dry, the preventive antibiotic was given before the incision, and the instrument tray's sterilization records were in order. Then a review of the case notes finds that midway through the operation, a glove was torn and replaced, but the surgeon kept working for several minutes before the tear was noticed. That is one possible route, from hand to wound. Another is endogenous: the patient's own skin flora, which preparation reduces but cannot remove completely. The team cannot prove which route it was, and in most surgical site infections no one can.

Where people slip

That uncertainty is the lesson. Because an infection usually cannot be traced to one moment, every link has to be guarded every time: clean instruments, prepared skin, intact gloves, and a team that speaks up the moment a break happens. Prevention works on all the links at once, because nobody will know afterward which one failed. A torn glove that is reported and changed at once is a small event; one that goes unnoticed becomes part of a story nobody can finish.
