Diagnosing a study failure means locating which memory operation broke — encoding, storage, retrieval, or the choice of practice method.
In practice
One student's failed exam worked through end to end: hours logged, method used, and the four separate failures found — rereading mistaken for encoding, massed sessions mistaken for enough time, recognition during study mistaken for recall, and two similar chapters studied back to back producing interference. Each failure named, and the corrected plan built from the same number of hours.
