The idea
Stimulus intensity is coded by how often neurons fire and how many neurons fire, never by the size of one spike.
In practice
A bright light produces faster firing across more optic nerve fibers than a dim one.
Where people slip
The error survives because intensity really is coded. The coding happens one level above the single neuron.
