Advance Directives

Captions available

The idea

An advance directive is a legal document stating a person's wishes about medical care if they become unable to decide for themselves.

In practice

It can also name someone to make decisions for them; forms and rules vary by state.

Where people slip

Ask whether the patient has an advance directive, and make sure a copy is in the record if the practice keeps one.
