The idea
Many plans require approval before certain tests, procedures, surgeries or medications, called preauthorization or prior authorization.
In practice
The request goes to the plan with the information it requires, and the plan approves or denies it.
Where people slip
Without required approval, the plan may refuse to pay, leaving the patient with the bill.
