Lock the Wheels

The idea
Before any transfer, lock the wheels of every piece of equipment involved.
In practice
Place the wheelchair next to the table or bed and set its brakes, and keep wheelchair wheels locked whenever it is not moving.
Where people slip
An unlocked chair can roll away at the moment the patient sits, which is how falls happen.
