Evidence-Based Practice / Unit 10   EBP 1381 short lesson, about 20 seconds

The Cause-and-Effect Diagram

Captions available

The idea

A cause-and-effect diagram, also called a fishbone, groups the possible causes of a problem into categories such as people, process, equipment and environment.

In practice

Unclear roles, no written guidelines and heavy workload sorted into branches under one problem.

Where people slip

The diagram organises hypotheses. It does not establish which cause is operating, and data still have to decide that.
