The idea
At the sign-out, the scrub person or circulating nurse confirms aloud that the final sponge, sharps and, where required, instrument counts are complete and correct.
In practice
If a count is not reconciled, the team searches the drapes, trash and wound, and an X-ray may be taken.
Where people slip
An unresolved count is resolved, or handled as policy directs, before the patient leaves.
