The idea
The scrub person tracks how much of each medication on the field has been used, so the record matches what the patient received.
In practice
Labels stay on every container, and the scrub person tells the circulating nurse how much was used when asked.
Where people slip
A medication that ran out mid-case is replaced through the same labelled transfer as at the start.
