Captions available
The idea
Assistive personnel are expected to report what they notice, such as a change in a patient or an unusual reading, to the licensed professional right away.
In practice
If a patient's breathing looks labored while you take vital signs, stop and tell the provider before going on.
Where people slip
A finding you notice but do not report is a finding nobody acts on.
Your place on the trail
