Hormone Therapy: How the Evidence Changed
The idea
Hormone therapy, estrogen alone or with progestin, was widely prescribed for menopausal symptoms in the nineteen nineties, often with the expectation that it would also protect the heart. Much of that expectation came from observational studies, in which women who took hormones had better heart health. The Women's Health Initiative tested the idea in large randomized trials, and in two thousand two a trial was stopped early over increased breast cancer and cardiovascular risks. Concern about these risks changed how often hormone therapy was prescribed. Later reanalysis added nuance: the risks and benefits depended on when women started. Starting within about ten years of menopause was associated with symptom relief and lower cardiovascular and mortality risk, while later starts carried more risk. Today hormone therapy is described as one option for symptoms, weighed by timing and individual risk.
In practice
Two fifty-two-year-olds a decade apart: one prescribed hormones routinely in nineteen ninety-eight, the other told in two thousand three that they were dangerous, and both told something more nuanced today.
Where people slip
The misconception is that the story is either "hormones are dangerous" or "hormones are safe." The lesson is about evidence. Observational studies suggested protection, perhaps because healthier women chose hormones: a third-variable problem. Randomized trials found harm in the population studied. Reanalysis by timing then qualified the harm. Each step was a reasonable reading of the evidence available at the time. The course describes this history; it gives no medical advice, and decisions about treatment belong to patients and clinicians. For the exam, know the Women's Health Initiative, the two thousand two halt, and the timing finding. The broader lesson applies to any treatment first judged by observational data.
