From an Irritation on a Shift to an Obligation

The idea

The technique: run the whole course on one question, from the moment something looked wrong on a shift to the point where doing nothing about it stopped being neutral.

In practice

Begin where video 3601 began, with three pressure injuries in a month where the previous quarter had one. Step zero was noticing. The question was built so a database could answer it, searched systematically, and the body of evidence appraised through four phases to a recommendation stating its own certainty. The numbers were read three ways, because a halving and three fewer patients in a hundred are the same finding landing differently. The three components met in a decision for one patient who valued sleep over a risk she understood. The outcome was defined before the change and measured after, on a run chart with a balancing measure. The change reached a unit that had to be given the infrastructure to carry it. It was taught where people work, carried into policy, disseminated so the next unit would not repeat it, and where evidence was missing, generated and funded. And it ends on ethics, because the four principles were present the whole way: beneficence in wanting the prevention, autonomy in the refusal, nonmaleficence in the harm the schedule itself could do, and justice in who gets the redistributing surface when there are not enough.

Where people slip

Not using evidence is an ethical matter, not merely a professional one. Doing nothing is not the safe option and never was, because the current practice also has effects — and after twenty-four units of method, that is the claim the whole course was built to earn the right to make.
