The idea
Whether clinicians can consistently practise from evidence depends on three organizational properties: the culture they work in, the organization's readiness, and the infrastructure available to them.
In practice
A capable clinician in a setting with none of the three, achieving nothing.
Where people slip
All three are organizational, not individual. Training a clinician into a setting that lacks them wastes the training.
