When Someone Else Breaks Technique

Captions available

The idea

When a colleague breaks technique, the scrub person says so, plainly and without accusation, whatever that person's role.

In practice

Follow the approach in order. First, say what you saw, specifically and calmly, such as that a sleeve touched the light handle. Second, say what needs to happen, such as a gown change. Third, offer what you can, such as having the replacement ready. Fourth, if the person disagrees, the rule still stands; involve the circulating nurse if needed. Fifth, after the case, if it matters, the event is reported through the facility's system, factually and without blame.

Where people slip

Speaking up to a senior colleague is hard. The textbook's definition of surgical conscience includes acting without fear of embarrassment or criticism, and a team that welcomes these moments is a safer team.
