Standards, Policies and Scope

The idea

Practice in surgery rests on three layers: published professional standards, the facility's written policies, and each role's scope of practice.

In practice

Here is how the three layers fit together. First, professional organizations publish evidence-based standards for perioperative practice, and textbooks, including the ones behind this course, teach them. Second, each facility writes policies that turn those standards into local procedures, and the textbooks repeat one instruction again and again: always follow the agency's policy. Third, scope of practice limits what each role may do. A surgical technologist in the scrub role does not obtain consent or give anesthesia, and an advanced role such as registered nurse first assistant needs added education and credentials. When the layers seem to disagree, you raise the question with the people responsible for the case rather than improvising.

Where people slip

This course teaches the standard. In a real operating room, facility policy, the surgeon and the circulating nurse decide.
